Subscription Application Requirements
A. Investing as a private individual(s), include beneficiaries if a trust and not in an editable format:
(i) Certified copy of current/valid passport(s) (clearly states name, date of birth, date of expiry)
(ii) Certified copy of proof of address(s) (utility bill, bank statement, driver’s license) – document less than 3 months old
B. Investing as a corporation the documents must be valid, final version and clearly states name of company and is not in an editable format:
(i) Names and addresses of all Directors
(ii) Certified copy of current/valid passports of all Directors
(iii) Names and address of all Owners with 10% or more interest
(iv) Certified copy of current/valid passport of all Owners with 10% or more interest
(v) Certified copy of Articles of Incorporation – only first page if document is bound
(vi) Certified copy of Certificate of Good Standing from the jurisdiction of incorporation
(vii) Copy of business license
(viii) Certified copy of board minutes authorizing investment
(ix) Certified copy of proof of address (utility bill, bank statement) – document less than 3 months old
Redemption Request Requirements
A. Must be clear and not in an editable format:
(i) Copy of current/valid passport(s)
(ii) Copy of FTM II contract note for shares being redeemed
(iii) Copy of voided bank cheque for account to which funds are to be deposited
